We would love to see you and your pet(s), and we’re happy to book an appointment for you using the form below. If you are a new client, please use our New Client Registration Form to schedule your first appointment. If you think your pet is experiencing an emergency, please call us: 425.486.3251. A few notes about appointments:

  • This is not an automatic process. Once we receive your request, we will contact you via email to confirm a date and time that is as close to your request as possible.
  • We are experiencing an unprecedented number of appointment requests at this time, so we may not be able to accommodate any last minute requests for you and your pet. We appreciate your understanding and patience.
  • If your appointment has not been confirmed within 48 hours, please feel free to contact the practice by phone to confirm at 425.486.3251.
  • Appointments are now subject to a cancellation fee if it is not cancelled within 48 hours of scheduled visit. Please do give us a call if you need to make any adjustments to your pet’s schedule. We are happy to assist you.

Professional Fees Are to Be Paid at the Time Services Are Provided

In admitting my pet(s) for diagnostics, treatment, or surgery, I authorize the veterinarians of Bothell Pet Hospital and their support staff to administer such treatment and/or perform such diagnostic or surgical treatment as deemed necessary. It is understood that I may request an estimate of charges for any services. No guarantee or assurance can be made as to the results that may be obtained. Furthermore, I realize that these charges may exceed a given estimate if complications arise. I understand that I will be contacted prior to treatment, if possible, should complications arise. I assume financial responsibility for all charges incurred for services provided and understand that full payment is required upon request.
